Affixed Labels
Product Details
Add-A-Label is our terminology for labels that are applied to forms. Also known as affixed labels, this is one of the oldest and most widely recognized methods of combining labels and forms. We offer unmatched flexibility in this product line. We can add nearly any type of label to any part of a form in a multitude of sizes and quantities.
Affixed Labels Value
The combination of a label and a form can greatly simplify and streamline your information flow. Instead of separate computer runs, all of the information is printed in one pass. This not only reduces printing time but also decreases the possibility of errors caused by mismatched documents while increasing processing productivity.

Frequently Asked Questions
Are forms with labels applied laser compatible?
Cut sheet forms with a label are not suitable for laser printers. Some laser printers are continuous and as such may be a candidate for a single ply form with a label applied. We have a number of other options that may be better suited for either application including integrated labels and our uni-web label/form.
How does an applied label work?
The label being applied is a two-ply label; the bottom ply is permanently applied to the form. The top ply is then peeled off and placed elsewhere as needed.
Where can the label be applied?
We have the ability to apply labels to any part of a form, including internal parts. We recommend that a label be placed at least 1/8” from any perforations on the form
What is the normal tolerance for positioning of the label?
Under most circumstances, placement of the label will be within 1/16” of the target area.
Do all applied labels have an imaging liner?
No, this is a special feature and should be requested during design and/or quoting.
What is a “tab slit”?
This is either a horizontal or vertical slit through the top ply of the applied label to allow for easier removal of the label from the form.
How about the cost of these forms?
Forms with a label applied will undoubtedly be more expensive than forms and labels purchased separately. However, when printing efficiency, accuracy and labor saving are calculated the true costs often show the label/form to be a good value.
